Abstract
After corneal transplantations, high astigmatism is an important complication limiting visual acuity. The major surgical factor contributing to astigmatism appears to be the difficulty in creating a truly round recipient opening. The reason is thought to be the distortion of the globe and the cornea resulting from the forces applied during manual trephination with current mechanical trephines.
